The Child Nutrition Office has shared the Free and Reduced-Price Policy Statement Survey with your SFA through the SED Monitoring and Vendor Performance System within the Business Portal. It is required that you complete and submit this documentation to the Child Nutrition office.

If you do not have an account, the Super Delegated Administrator (SDA) for your SFA will need to entitle and/or create an account for the person who will be responsible for uploading the Free and Reduced-Price Policy Statement. Instructions on granting entitlements for the SED Monitoring and Vendor Performance System are available here:
